Request an Appointment

Before Your First Visit

To save time on the day of your appointment, please complete your new patient paperwork in advance. You can download our Health History Form and, if you would like us to obtain records from a previous provider, our Records Release Form. Forms are also available through our secure patient portal.

Please bring the following to your first appointment:

  • A photo ID and your current insurance card
  • A list of medications, vitamins, and supplements you take, with doses
  • Any previous medical records, recent lab work, Pap smear results, or mammogram reports
  • The date of your last menstrual period
  • A list of questions or concerns you would like to discuss

We recommend reviewing our insurance page and confirming your benefits with your carrier before your visit.

What to Expect at Your First Appointment

Your visit begins with a review of your health history and vital signs. Then you will sit down with your provider to talk about your goals and concerns. Depending on the reason for your visit, your appointment may include a well woman exam, Pap smear or HPV test, breast exam, contraception counseling, or lab work. If you are pregnant, your first obstetric visit typically includes an ultrasound and prenatal labs.

We take the time to listen and explain your options clearly, so you never feel rushed. Before you leave, we will make sure you understand any next steps and schedule follow-up care if needed.
